Position Summary
The Service Navigator is the first point of connection for program participants, guiding them through intake, assessment, planning, and ongoing support. This role ensures that clients receive the life stability services, employment readiness tools, and resource connections they need to succeed. The Service Navigator works closely with families, community partners, and the team to build holistic, individualized plans that remove barriers and improve employment outcomes.
Key Responsibilities
Conduct Eligibility & Risk Assessments (ERA) to determine client program/service eligibility.
Work with clients to prepare personalized Action Plans, coach them on progress, and consult with management on concerns or high-needs cases.
Enter and track client activities in Compass and ensure accurate service documentation.
Provide life stability planning and barrier reduction support (housing, childcare, income supports, etc.).
Deliver warm referrals and assist clients in navigating community resources, tracking achievement of life stability goals.
Manage client services by tracking progress, addressing challenges, and advocating on behalf of clients when needed.
Develop and maintain relationships with community organizations to enhance support networks and resource access.
Provide clients with personalized roadmaps of local resources to improve their readiness for employment.
Facilitate opportunities for clients and community partners to network, share information, and build connections.
Qualifications
Post-secondary education in human services or a related field.
Minimum 2 years of demonstrated experience working with underrepresented job seekers, single parents, or families.
Familiarity with trauma-informed practice and family-centered supports.
Experience in life stability barrier identification and reduction (housing, childcare, income supports).
Knowledge of wraparound models and crisis response.
Skilled in goal-setting, referrals, and maintaining client engagement.
Excellent working knowledge of programs, services, and community resources.
Experience facilitating networking opportunities for clients and community partners.
Strong interpersonal, communication, and problem-solving skills.
